脑脊液置换治疗蛛网膜下腔出血的疗效分析

目的探讨大剂量脑脊液置换法治疗蛛网膜下腔出血的优势。方法在常规治疗的基础上 ,行腰椎穿刺待颅内压力 <3kPa以下缓慢放出血性脑脊液 5 - 10ml,缓慢注入等量生理盐水 ,如此反复置换 ,每次 2 0 - 6 0ml。结果治愈率治疗组为 98% ;对照组为 80 % ,X2 =6 5 7,,P <0 0 5。结论大剂量脑脊液置换法配合激素应用可防止脑积水、脑血管痉挛、癫痫等并发症 ,提高治愈率。

The clinical effects of cerebrospinaI fluid displacement on the treatment of subarachnoid hemorrhage

Objective To evaluate the effectiveness of cerebrospinal fluid displacement on the treatment of subarachnoid hemorrha.Methods Besides medicine treatment, spinal puncture were done to each patient in treatment group when their intracranial pressures were below 3 kap. Hemorrhagic cerebrospinal fluid were drained 5-10ml,and then infused same quantity of normal saline slowly.Repeat this operation several times, but the maximum quantity of drainage cerebrospinal fluid were 20-60 ml. Results The cure rate of 98% in treatment group compared to the cure rate of 80% in control group by Chi-square test (X2=6.57, P<0.05).Conclusion Cerebrospina! fluid displacement can prevent the complications of subarachnoid hemorrhage such as hydrocephalus, cerebral vasospasm and epilepsy et al and improve the cure rate of subaracknoid hemorrhage.
