多次人工流产术后并发症的预防研究

目的:研究多次人工流产术并发症的预防措施。方法:将我所行第3次人工流产术的早孕妇女90例,随机分为研究组和对照组,每组45例,研究组术前3h口服米非司酮片50mg,同时阴道放置米索前列醇400μg;对照组采用传统的人工流产术。随访和观察术中、术后阴道出血量及出血时间、术后月经复潮时间和量的变化、腹痛症状、宫腔镜检查(术后3个月)有无粘连及程度。结果:研究组中术中出血量、术后阴道出血时间、月经复潮时间均短于对照组,两组比较差异有显著性(P<0.05),3个月后宫腔镜结果显示:研究组中宫腔粘连的总发生率及轻、中度粘连发生率低于对照组,差异有显著性(P<0.05)。结论:在人工流产前联合使用米非司酮及米索前列醇,大大降低了人工流产术的近期和远期并发症,特别是宫腔粘连的发生,值得临床推广应用。

The Study of the Complications Precautions after Multiple Artificial Abortions

Objective:To study the complications precautions of multiple artificial abortion.Methods:Randomly dividing 90cases of early pregnant women,whom I did the third artificial abortion surgery for,into study group and control group,45cases each.In the study group,50mg oral mifepristone tablets was taken 3hours before the operation,and at the same time 400μg of misoprostol was put into vagina.In the control group,I did in the traditional way.Then following-up and observing the amount of intraoperative and postoperative vaginal bleeding and the bleeding duration,as well as the time of the next menses and the changes of the amount,abdominal pain,hysteroscopic result(after 3 months),whether the adhesions exist and the extent.Results:The amount of intraoperative bleeding,the postoperative vaginal bleeding duration,and the interval before the next menses of the study group were less or shorter than the control group,and the differences were significant(P<0.05),what is more,the results of hysteroscopy 3months after surgery show that:the incidence of intrauterine adhesions,the incidence of light and moderate adhesions of the study group were lower than the control group signigicantly(P<0.05).Conclusion:The combined use of mifepristone and misoprostol in artificial abortion could be considered to greatly reduce the near-term and long-term complications of artificial abortion,and the incidence of intrauterine adhesion,which deserves further clinical applications.
