| 摘 要： | 目的探讨医患直接沟通联合远程随访对社区慢性心力衰竭患者的影响。方法将慢性心力衰竭患者228例随机分成A组（医患直接沟通进行管理）及B组（医患直接沟通基础上,接受远程管理）;所有心力衰竭患者均按常规治疗。观察所有患者不良生活方式改变情况、遵医服药情况、住院次数、再住院率、及医疗费用。结果 B组与A相比,在生活方式改良、遵医行为、住院次数、医疗费用等方面差异均有统计学意义（P〈0.05）;由于观察时间短,是否能降低病死率有待进一步观察。结论通过医患直接沟通基础上,接受远程管理对慢性心力衰竭患者的管理,能提高患者对治疗的依从性,降低再入院率及医疗费用。
