剥脱综合征行超声乳化人工晶状体植入的评价

目的　评价剥脱综合征眼行白内障超声乳化人工晶状体植入术的安全性。方法　对剥脱综合征 6例 (8眼 )及 5 0岁以上单纯白内障 60例 (66眼 )行超声乳化人工晶状体植入术 ,观察分析术中并发症的发生情况及术后眼部变化。结果　剥脱综合征术中无后囊破裂 ;非剥脱综合征眼发生后囊破裂 1眼 ,占 1 5 2 %。两组间差异无显著意义 (P >0 0 5 )。剥脱综合征眼术中无晶状体悬韧带断裂 ;非剥脱综合征眼发生 1眼 ,占 1 5 2 %。两组间差异无显著意义 (P >0 0 5 )。两组病例均未发生术后人工晶状体移位 ,亦未发现剥脱综合征术后眼内反应明显重于非剥脱综合征眼。结论　剥脱综合征眼行超声乳化人工晶状体植入术是安全的。术前充分认识及估计术中的风险 ,谨慎操作 ,就可以避免各种并发症的发生。

Evaluation of phacoemulsification in patients with exfoliation syndrome

Objective To evaluate the safety of phacoemusi fication and intraocular lens implantation in patients with exfoliation syn drome.Methods Phacoemusification and intraocular lens(IOL) implantation was performed on 8 eyes of 6 patients with exfoliation syndrome and 66 eyes of 60 patients over 50 years old with senile cataract. All operations were completed by one experienced surgeon. The intraoperative and postoperative complications in two groups were analysed. Results In both two groups the occurrence of posterior capsular tear were 0 and 1.52% respectively (P>0.05); the occurrence of zonular dehiscence were 0 and 1.52% respectively(P>0.05); no eye had phacodonesis in either groups;there was no difference in severity of intraocular inflammation after surgery between two groups. Conclusion Although the exfoliation syndrome increase the risk of intraoperative complications in cataract surgery, the phacoemulsification is safe and reliable. The risk must be realized before the surgery and be very cautious with the operation, the intraoperative complications can be avoided. When glaucoma is accompanied, the anti-glaucoma treatment is need sim ultaneously.
