手法复位治疗急性腰椎间盘突出症的临床研究

目的探讨手法复位对急性腰椎间盘突出症的疗效并观察体感诱发电位在病情评估中的作用。方法将48例急性腰椎间盘突出患者随机分为手法复位治疗组24例和常规治疗组24例,治疗前及治疗后3d、1周及出院前应用目测类比评分法(VAS)对疼痛进行评估,治疗前后应用腰椎疾患治疗成绩评分表对腰椎功能状态进行临床评定,并比较治疗前后下肢体感诱发电位的变化。结果手法复位可迅速缓解疼痛,疗效显著,治疗优良率为79.1%,平均改善指数及改善率均高于常规组(P<0.05)。48例患者下肢体感诱发电位异常率79.2%,主要表现为马尾电位潜伏期延长及周围神经传导速度减慢,治疗后均有改善。结论手法复位对急性腰椎间盘突出症有较好的临床疗效,体感诱发电位可作为腰椎间盘突出症病情及疗效评定的指标。

The clinical study of manual reduction on acute lumbar disc herniation

Objective To explore the therapeutic effect of manual reduction on acute lumbar disc herniation and to observe the somatosensory evoked potential on the assessment of state of the illness. Methods 48 cases of acute lumbar disc herniation were divided into the manual reduction treatment group 24 cases and the traditional treatment group 24 cases. The clinical symptoms and signs were assessed before and after treatment.The effect of treatment and the low limb somatosensory evoked potential were assessed. Results The manual reduction can restrain pain rapidly.The eminent rates were 79.1%. The index and rate in improvement and efficiency of peridural injection group is higher than traditional group (P< 0.05). The abnormity of SEP was 79.2%. The main manifestation were delay of latent period and decrease of nerve transmit velocity. Both of two indexes were improved after treatment in the two groups.Conclusions The manual reduction has preferable clinical curative effect. The somatosensory evoked potential can be an index of the state of illness and curative effect of acute lumbar disc herniation.
