以埃索美拉唑为中心的三联疗法根除幽门螺杆菌的疗效观察

目的观察埃索美拉唑、克拉霉素、阿莫西林1周疗法根除幽门螺杆菌（Hp）的疗效。方法选择82例Hp阳性消化性溃疡患者，随机分为观察组42例，对照组40例。观察组采用埃索美拉唑（20mg，2次／d）加克拉霉素（500mg，2次／d）加阿莫西林（1000mg，2次／d），治疗7d后继续用埃索美拉唑20mg，1次／d，连用4周。对照组采用奥美拉唑（20mg，2次／d）加克拉霉素（500mg，2次／d），加阿莫西林（1000mg，2次／d），治疗7d后继续用奥美拉唑20mg，1次／d，连用4周。疗程结束4周后复查胃镜和Hp检查，并观察症状缓解率、溃疡愈合率和Hp根除率。结果治疗组和对照组的症状迅速缓解率分别为95．24％和80．00％，两组比较，差异有显著性（P〈0．05），溃疡愈合率分别为92．86％和87．50％，两组比较差异无显著性（P〉0．05），两组的Hp根除率分别为95．24％和77．50％，差异有显著性（P〈0．05）。结论埃索美拉唑、克拉霉素、阿莫西林三联药物合用，Hp根除率优于对照组，能有效控制消化性溃疡、促进其愈合、迅速缓解其临床症状，安全且耐受性好，是一种较为理想的治疗方案。

Clinical analysis of triple therapy of combined for Helicobacter Pylori eradication

Objective To observe the effect of 1 week triple therapy of esomeprazole, Clarithromycin and amoxicillin combined for Helicobacter pylori （Hp） eradication. Methods 82 patients with Hp infection Sufering from Peptic ulcer were randomized to observer group 42 cases and control group 40 cases. The observer group were administered esomeprazole （20 mg, twice a day） , Clarithromycin （500 mg, twice a day）and amoxicillin（ 1000 mg, twice a day）for 1 week and the patients were treated with esomeprazole （20 mg, every day）for another four weeks. The control group were administered omeprazole （20mg, twice a day） , Clarithromycin （500 rag,twice a day）and amoxicillin （1000 mg, twice a day）for 1 week and the patients were treated with ome- prazole （20 mg, every day） for another four weeks. Four weeks later, the Hp eradication, symptoms, peptic ulcer healing and side-effects were observed. Results The symptom ameliorate rate between the observer group and control group were 95.24% , 80. 00% , respectively. There was significant difference between two groups （ P 〈 0. 05 ）. puptie ulcer healing rate between the observer group and control group were 92.86% and 87.50% ,The diference was not significant （ P 〉 0. 05 ）. The Hp eradication rate between the observer group and control group were 95.24% ,77.50%, respectively. There was significant difference between two groups （P 〈 0. 05 ）. Conclusion 1 week triple therapy of esomeprazole, Clarithromycin and amoxicillin combined is the first choice for Hp eradication. It＇ s effective control of peptic ulcer, promote ulcer healing, quickly alleviate the clinlcal symptoms, safe and tolerated. It＇ s anideal choice for clinical.
