洛伐他汀对冠心病患者血浆可溶性细胞间黏附分子-1和P-选择素水平的影响

摘    要:目的观察洛伐他汀对冠心病(CHD)患者血浆可溶性细胞间黏附分子-1(sICAM-1)及可溶性P-选择素水平的影响,探讨洛伐他汀调脂外的抗炎作用。方法将99例CHD患者分为基本治疗组(对照组)与洛伐他汀 基本治疗组(治疗组),经8周治疗,两组分别于治疗前、后采用全自动生化分析仪测定血脂浓度,应用酶联免疫吸附试验(ELISA)检测血浆sICAM-1和可溶性P-选择素水平,治疗后随访心血管发生事件3个月。结果治疗组治疗后与治疗前比较胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、甘油三脂(TG)、载脂蛋白B(ApoB)均有明显下降、高密度脂蛋白胆固醇(HDL-C)、载脂蛋白A(ApoA)明显升高,血浆sICAM-1和可溶性P-选择素水平显著下降(P<0.05);对照组治疗前后差异无显著性(P>0.05);冠状动脉事件发生治疗组2例(4%),对照组7例(14.3%),治疗组较对照组明显减少。结论洛伐他汀降低血脂的同时具有明显的降低血浆sICAM-1和可溶性P-选择素水平的抗炎作用,并能减少心血管事件的发生。

Abstract:Objective To observe the effect of lovastatin on plasma levels of soluble intercellular adhesion molecule-1 and soluble P-selectin in patients with coronary artery disease and to investigate its action on non-lipid anti-inflammation mechanisms.Method Ninety-nine patients with coronary artery disease(CHD) were randomly assigned into two groups: a control group with basic treatment and a treatment group with basic treatment plus lovastatin.All patients were treated for eight weeks.Before and after the treatment,blood lipids were measured by auto-chemistry analysis detector.Plasma soluble intercellular adhesion molecule-1 and soluble P-selectin were measured by enzyme-linked immunosorbent assay(ELISA).Follow-up was conducted for three months.Results After the treatment,the serum levels of cholesterol(TC),low dense lipoprotein cholesterol(LDL-C),triglyceride(TG) and apolipoprotein(ApoB) in treatment group declined significantly;serum high dense lipoprotein cholesterol(HDL-C) and apolipoprotein(ApoA) increased significantly;while plasma soluble intercellular adhesion molecule-1 and P-selectin decreased significantly(P<0.05).There showed no difference between before and after treatment in control group(P>0.05).Coronary events occurred in 2 cases of the treatment group(4%) and 7 cases of the control group(14.3%),and the incidence in treatment group was significantly lower than that of control group.Conclusion The results suggest that lovastatin has non-lipid anti-inflammatory effect on inhibiting plasma soluble intercellular adhesion molecule-1 and P-selectin,which can reduce coronary events in patients with CHD.
