224例腹膜透析相关性腹膜炎的临床分析

目的分析我院2006年1月至2012年6月腹膜透析相关性腹膜炎发生的相关情况,以指导腹膜炎的预防和治疗。方法回顾性分析我院2006年1月至2012年6月维持腹膜透析患者腹膜透析相关性腹膜炎的基本情况、培养结果及可能的致病因素。结果我院160名腹膜透析患者共发生224次腹膜透析相关性腹膜炎,腹膜透析相关性腹膜炎的发生率为20患者月/次,其基础疾病以慢性肾炎为主。培养结果中,革兰阳性菌以金黄色葡萄球菌及表皮葡萄球菌常见,革兰阴性细菌以大肠埃希菌及鲍曼不动杆菌常见。影响腹膜透析相关性腹膜炎发生的常见原因为不注意无菌操作。结论加强腹膜透析操作的培训对预防腹膜透析相关性腹膜炎有重要意义。有必要对维持腹膜透析患者进行腹膜透析操作再培训。

Clinical Analysis of 224 Patients with Peritoneal Dialysis-related Peritonitis

Objective To analyze the correlation factor of patients with peritoneal dialysis-related peritonitis in our hospital from January 2006 to June 2012,we try to find a way to prevent and cure the peritonitis.Methods We retrospectively analyzed the basic information,result of culture and possible etiological factor of patients with peritoneal dialysis-related peritonitis in our hospital from January 2006 to June 2012.Result 160 patients had occurred peritoneal dialysis-related peritonitis 224 episodes in our hospital in the period.The incidence rate of the peritonitis was 1 episode every 20 months.The underlying disease is primary chronic nephritis.The culture result of the effluent showed that Staphylococcus aureus and Staphylococcus epidermidis were the most common Gram-positive bacteria,while Escherichia coli and Acinetobacter baumannii were most common gram negative bacteria.The etiological factor that affected peritonitis mostly was absence of sterile working.Conclusion Reinforcement of operation of peritoneal dialysis has important significance in preventing the occurrence of peritonitis.It is worth to retraining patients with peritoneal dialysis on operation.
