食管癌和贲门癌吻合口瘘的防治

目的：探讨食管癌和贲门癌术吻合口瘘发生的因素和防治。方法：手术切除肿瘤，行食管胃吻合。本组共279例，其中胸腔内吻合者208例，颈部吻合者71例。结果：胸腔内吻合口瘘8例，死亡4例。颈部吻合口瘘10例，无1例死亡。结论：注意手术操作及改进吻合方法是减少吻合口瘘的关键因素，确切的感染控制和充分的营养支持是降低死亡率的有效手段。

Prevention and treatment of anastomotic fistula of esophagus cancer and cardiac cancer

Objective:To study the generative factors of anastomotic fistula after esophagus cancer and cardiac cancer operation and its prevention and treatment.Methods:After excising the turnors,the esophagogastrostomy was performed in 279 cases aged 37 to 78 years,in which the intrathoracic anastomose was adopte in 208 cases and cervial anastomose in 71 cases.Results:Four of 8 patients with anastomotic fistula in the intrathoracic anastomse died,while none of 10 cases in the cervial anastomose died.Conclusion:Careful procedure and improving the anastomotic method are the key factors to reduce the anastomotic fistula,the accurate control of infection and full nutritive support are the effective methods to decrease the mortality rate of this disease.
