| 摘 要： | 为提高对纯红细胞再生障碍性贫血（ＰＲＣＡ）发病机制、诊断和治疗的认识，对３７例ＰＲＣＡ住院患者进行回顾性分析。免疫学检查２８例，１７例有体液或细胞免疫方面的异常，其中循环免疫复合物（ＣＩＣ）阳性８例。对１４例骨髓红系祖细胞（ＣＦＵ－Ｅ）作体外培养，其中８例ＣＦＵ－Ｅ数量正常；６例患者未灭活血清对自身ＣＦＵ－Ｅ产生明显抑制，而灭活血清未出现抑制。说明ＰＲＣＡ的发生可能与患者血清中一种依赖补体的抑制物对ＣＦＵ－Ｅ或红系前体细胞抑制有关。２７例ＰＲＣＡ使用泼尼松或泼尼松加雄激素治疗，１７例有效，两种治疗方案疗效相同。证明单用泼尼松治疗ＰＲＣＡ可取得与雄激素并用同样疗效，并可避免雄激素引起的副作用

| Abstract: | Thirty-sevenhospitalizedpatientswithpureredcelaplasia(PRCA)wereanalysedretro-spectively.Amongthem,17casesshowedabnormalresultsofimmunologicaltestand8hadcirculationimmunecomplexes.BonemarrowCFU-Ewereculturedin14cases,and8ofthemshowednormalre-sults.In6casesthegrowthofCFU-Eweresignificantlyinhibitedbytheirownuninactivatedbutnotbyinactivatedsera.Theseresultssuggestedthatcomplement-dependentinhibitorstoCFU-EorerythroidprecursorsmayberelatedtothegenesisofPRCA.Tweenty-sevenPRCApatientsweretreatedwithprednisonealoneorincombinationwithandrogenand17responded.Prednisonealonehadasgoodresponseasprednisonecombinedwithandrogen. |
