牙周炎患者血清及龈沟液中多项炎性细胞因子的变化观察

目的:观察牙周炎患者血清及龈沟液中多项炎性细胞因子水平的变化。方法:选择慢性牙周炎患者50例作为牙周炎组,纳入同时期口腔健康志愿者50例作为对照组。牙周炎组患者进行超声龈上洁治、龈下刮治和根面平整等牙周基础治疗。比较基线时两组患者间和牙周炎组治疗前后牙周状态相关指标(牙周探诊深度(probing depth,PD)、龈沟出血指数(sulcus bleeding index,SBI)、临床附着水平(clinical attachment level,CAL)、血清和龈沟液中炎症因子TNF-α、IL-1β、IL-5、IL-6、IL-8水平差异。结果:基线时牙周炎组牙周状态相关指标PD、CAL、SBI均显著高于对照组(P<0.05)。牙周炎组患者治疗后1、3个月PD、CAL、SBI与基线相比均显著下降(P<0.05)。基线时牙周炎组患者血清及龈沟液中炎症细胞因子TNF-α、IL-1β、IL-5、IL-6、IL-8均显著高于对照组(P<0.05)。治疗后1、3个月,牙周炎组患者血清及龈沟液中炎症细胞因子TNF-α、IL-1β、IL-5、IL-6、IL-8与基线相比均显著降低(P<0.05)。结论:牙周炎患者血清及龈沟液中炎性指标明显升高,通过牙周基础治疗能有效下调炎性细胞因子水平,改善其牙周状态。

Observation of the change of various inflammatory cytokines in serum and gingival crevicular fluid of patients with periodontitis

Objective:To observe the change of various inflammatory cytokines in serum and gingival crevicular fluid of patients with periodontitis. Methods:Patients with chronic periodontitis in our hospital were selected as periodontitis group,50 cases of oral health volunteers in the same period as control group. Patients in periodontitis group took periodontal therapy such as ultrasound gingival scaling, subgingival scaling and root planing,etc. Then the indicators of periodontal status such as PD,CAL,SBI and the parameters of inflammato-ry cytokines such as TNF-α,IL-1β,IL-5,IL-6,IL-8 between two group and before and after treatment were compared. Results:The indica-tors of periodontal status such as PD,CAL,SBI of periodontitis group were all significantly higher than control group at the baseline( P<0. 05). And the serum and gingival crevicular fluid levels of TNF-α,IL-1β,IL-5,IL-6,IL-8 of periodontitis group were all significantly higher than control group at the baseline(P<0. 05). After 1,3 months of treatment,the serum and gingival crevicular fluid levels of TNF-α,IL-1β,IL-5,IL-6,IL-8 of periodontitis group were all significantly reduced compared to baseline( P<0. 05). Conclusion:The serum and gingival crevicular fluid levels of inflammatory cytokines such as TNF-α,IL-1β,IL-5,IL-6,IL-8 of patients with periodontitis was in-creased significantly,and periodontal therapy can effectively improve the inflammation and periodontal status and down the levels of in-flammatory cytokines.
