急诊体外循环在创伤性心脏和大血管损伤中的应用

摘    要:目的 临床评价急诊体外循环(emergency extracorporeal circulation,EECC)抢救和治疗创伤性心脏、大血管损伤危重患者的效果.方法 应用EECC抢救和治疗创伤性心脏、大血管损伤危重患者13例.其中交通伤致胸主动脉破裂2例,介入治疗时发生创伤性心脏、大血管损伤5例;手术中发生心脏、大血管损伤6例.结果 13例全部存活,1例应用深低温停循环者术后出现下肢远端部分麻痹.术后随访1~5年,生存良好.结论 加强围术期处理,尽早建立EECC,改良EECC技术,可提高抢救和治疗创伤性心脏、大血管损伤危重患者成功率.

Abstract:OBJECTIVE To evaluate the effect of emergency extracorporeal circulation in emergency treatment of cardiovascular trauma patients. METHODS A total of 13 imminent patients with cardiovascular trauma underwent emergency extracorporeal circulation . Among these cardiovascular trauma, 2 ruptured thoracic aorta occurred in traffic accidents; 5 trauma of hearts and vascular occurred during intervenient treatment; and 6 occurred in operating theater. RESULTS All patients were alive. 1 patient suffered from pelvic limb distal end paralysis after profound hypothermia and suspension of CPB. Following up from 1 to 5 years and they were all lived well. CONCLUSION To establish emergency extracorporeal circulation as early as possible and upgrade CPB technique can improve the survival rate of emergency treatment of cardiovascular trauma patients.
