Crede排尿法在脊髓损伤患者康复期的应用

目的:探讨脊髓损伤患者膀胱功能障碍的康复效果。方法:收集2003年1月至2007年1月住院的脊髓损伤伴神经源性膀胱功能障碍63例,分别施以Crede法排尿、间歇导尿和留置导尿等膀胱功能训练;定期尿液培养,进行尿路感染率比较;治疗后检测膀胱容量和残余尿量。结果:治疗3个月,Crede法排尿、间歇导尿和留置导尿3组尿路感染率分别为14.3%、28.6%和42.9%,差异有统计学意义(P<0.05)。膀胱容量Crede法排尿和间歇导尿组明显大于留置导尿组,残余尿量Crede法排尿组少于间歇导尿和留置导尿组。结论:脊髓损伤后应尽可能早期拔除导尿管,进行Crede法排尿,有利于膀胱功能的恢复及减少泌尿系感染的发生率,同时免除了因留置导尿管所造成的忧郁心理,从而提高患者的生活质量。

The Crede urination law the application in the spinal cord is restored

Objective:Discusses the spinal cord to damage the patient urinary bladder function barrier the recovery nursing effect.Methods:Collected from January,2003 the spinal cord which was hospitalized to January,2007 to damage the partner nerve source urinary bladder function barrier patient total 63 examples to serve with Crede urinary bladder function trainings and recovery nursing separately and so on law urination,intermittent catheterization and leaving alone catheterization,the regular urine raise carried on the urine road infection percentage comparison,after the treatment examines the urinary bladder capacity and the remaining amount of urine.Results:63 example patients treat for 3 months,Crede law urination,intermittent catheterization and the leaving alone catheterization three group of urine road infection percentage respectively are 14.3%,28.6% and 42.9%,the difference has statistics significance(P<0.05),the urinary bladder capacity Crede law urination and the intermittent catheterization group is bigger than the leaving alone catheterization group obviously,the remaining amount of urine Crede law urination group is short in the intermittent catheterization and the leaving alone catheterization group.Conclusion:After the spinal cord damage,should the early wiping out catheter,carry on the Crede law urination as far as possible,is advantageous in the urinary bladder function restoration and the reduced uropoiesis is the infection formation rate,because simultaneously avoided has left alone the melancholy psychology which the catheter created,improved patient's quality of life.
