药物治疗与腹腔镜保守性手术治疗输卵管妊娠的疗效及预后分析

目的探讨药物治疗与腹腔镜保守性手术治疗输卵管妊娠的疗效及预后。方法回顾性分析输卵管妊娠保守性治疗病例,按保守性药物、腹腔镜下保守性手术2种不同的治疗方案分组,比较2组治愈率及治疗后输卵管的通畅率、宫内妊娠率、再次异位妊娠率。结果保守性药物组与腹腔镜下保守性手术组治愈率分别为79.3%和97.3%,有显著性差异;治疗后输卵管通畅率分别为54.3%和88.9%,有非常显著性差异;宫内妊娠率分别为47.8%和65.3%,有显著性差异;再次异位妊娠率分别为10.9%和11.1%,无显著性差异。结论对于输卵管妊娠的保守性治疗,腹腔镜下保守性手术治愈率、治疗后输卵管的通畅率、宫内妊娠率均高于药物治疗。对于HCG较低且血流动力学稳定的患者可采用药物治疗。
