负压吸宫术前阴道放置米索前列醇与利多卡因胶浆对宫颈扩张作用的比较

目的:比较在终止早期妊娠负压吸宫术前阴道放置米索前列醇与利多卡因胶浆的宫颈扩张作用、人工流产综合征发生率、阴道出血量及副反应。方法:将停经6～10周要求手术终止妊娠的196例妇女随机分两组,米索前列醇组阴道放置米索前列醇400μg,用药后3h 行负压吸宫术,利多卡因组阴道内放置利多卡因胶浆5ml(内含利多卡因0.2g),用药后10min 行负压吸官术。记录观察对象的宫颈扩张程度、人工流产综合征发生情况、用药后副反应、术中出血量、手术所需时间。结果:两组对象宫颈扩张程度、手术所需时间等无显著差异,米索前列醇组用药后副反应、人工流产综合征发生率大于利多卡因组(P<0.01),利多卡因组阴道出血量增多者多于米索前列醇组(P<0.01)。结论:终止早期妊娠的负压吸宫术前阴道放置利多卡因胶浆与米索前列醇扩张宫颈效果相似,但前者用药后副反应少,人工流产综合征发生率低,使用方法简便、安全,等待受术时间短,缺点是术中阴道出血量偏多。

Comparision of Vaginal Misoprostol Application with Lidocaine Hydrochloride Mucilage on the Facility of Cervix Dil-atation in Surgical Abortion

Objectve:To compare the effect of vaginal misoprostol application with lidocaine hydrochloride mucilage on the fa- cility of cervix dilatation,the abortion syndrome rate,vaginal bleeding amount and side effect in a surgical abortion.Methods: 196 women opting for voluntary surgical abortion were randomly allocated to 2 groups.Group A received misoprostol and the operation began 3 hours later;group B received lidocaine hydrochloride mucilage and the surgical abortion started 10 minutes later.The servix dilatation,the abortion syndrome rate,side effect,the amount of vaginal bleeding and the needed period of a- bortion were observed.Results:there was not much difference between two groups in cervix dilatation and operation time.Group A had more side effect and a bigger abortion syndrome rate than group B(P<0.01);More women in group B suffered a larger amount of vaginal bleeding than in group A(P<0.01).Conclusion:During the course of pregnancy termination,vaginal lido- caine hydrochloride mucilage application has similar effect with misoprostol in cervix dilatation,but lidocaine application has the advantage of less side effect and low surgical abortion syndrome rate,and it can be adopted easily and safely,with a much shorter waiting period before the operation.The defect is a larger amount of vaginal bleeding.
