腰椎间隙感染的MRI早期诊断价值探讨

目的总结分析腰椎间隙感染的早期MRI影像学特性，探讨MRI对早期腰椎间隙感染的诊断价值。方法回顾分析8例经临床手术病理证实的腰椎间隙感染的MRI表现。结果8例患者中有11个椎间盘感染，其中其中腰3．4椎间隙受累1个，腰4．5椎间隙受累7个，腰5骶1椎间隙受累3个。MRI表现为感染椎间隙变窄，椎间盘不同程度的破坏、碎裂，呈长T1长亿信号，12聊上髓核内正常裂隙状短12信号消失，相邻椎体终板及终板下骨质破坏，椎旁软组织肿胀。3例行增强MRI扫描，病变椎间盘及邻近椎体、椎旁软组织明显强化。结论MRI对诊断腰椎间隙感染具有较高的敏感性和特异性，可准确鉴别本病与椎体结核与肿瘤，在临床工作中应作为首选的影像学检查方法。

The discussion of MRI'S value cn the diagnosis of Inter vertebral infection

Objective To analyze MRI features of intervertebral infection and evaluate MRI value in the diagnosis of intervertebral infection.Methods 8 patients with clinically or pathologically proven intervertebral infections underwent the spine MRI.Results 11 intervertebral infections were detected in 8 patients. Among the 11 intervertebal infections, there are 1 being invovled in the infection of the intervertebal space between the third＇s and fourth lumbar spine,7 being invovled in the fourth＇ s and fifth＇ s lumbar spine and 3 being incovled in the infection of the intervertebal space between the fifth sacro and first lumbar spine. The lesion sites of 11 intervertebral infections were lumbar discs. Sagittal T1-and T2-weighted images, T2-weighted images with fat-suppression and axial T2- weighted images were obtained in all cases, and contrast enhanced MR/were ordered in 3 patients. MRI findings disclosed that in 3 patients, abnormal enhancement of the diseased discs, adjacent vertebral bodies and paravertebral soft tissues were found after MRI administration. Conclusion Compared with plain film or CTscanning, MRI is a technique with high sensitivity and accuracy for diagnosing intervertebral infection, and it is recommendable as the first choice of imaging procedure in the diagnosis of intervertebral infection.
