晶状体后囊膜及局部玻璃体切割预防后发性白内障

目的　探讨Ⅰ期晶状体后囊膜及局部玻璃体切割术预防后发性白内障的作用。方法　 65例 (69眼 )随机分为两组 ,实验组 3 8例 3 8眼常规白内障摘出人工晶状体植入术中 ,行Ⅰ期晶状体后囊膜及局部玻璃体切割术 (切割组 ) ,对照组行I期后囊膜切开术。术后观察后囊切口混浊情况。结果　随访 1～ 2年 ,切割组 3 8例 (3 8眼 )均保持视轴清晰 ,无一例发生后发性白内障 ,有效率 10 0 % ;对照组 3 1例 (3 1眼 ) ,19例后囊切口未闭合 ,且切口区透明 ,有效率61 2 9%。切割组明显优于对照组 (P <0 0 1)。结论　晶状体后囊膜及局部玻璃体切割能有效预防后发性白内障。

Posterior capsulotomy and anterior vitrectomy preventing after-cataract

ObjectiveTo study the effect of the posterior capsulotomy and anterior v itrectomy on preventing the occurrence of after-cataract.MethodsSixty-five cases (69 eyes) with cataract were divided into two groups,38 eyes underwent e xtracapsular cataract extraction (ECCE) and posterior chamber intraocular lens i mplantation (pc-IoL)combined with primary capsulotomy and anterior vitrectomy,t he other 31 eyes received the ECCE combined with posterior capsulotomy as the c ontrol.After operation,the degree of opacification of posterior capsule were obs erved.The follow-up period was 12 years.ResultsAll the patients of the study group kept visual axial transparent during follow-up.The effective ra te was 100%in vitrectomy group,and 61 29% (19/31) in control group.The differen ce was significant between two groups ( P <0 01).ConclusionPosterior cap sulotomy and anterior vitrectomy can prevent the occurrence of after-cataract e ffectively.
