妇科宫腔镜与腹腔镜联合检查与手术29例病例分析

目的　探讨宫腔镜腹腔镜联合检查与手术的适应证和优点。　方法　回顾性分析妇科 2 9例应用宫、腹腔镜联合检查与手术的病例。　结果　检查结果与术前诊断符合 18例 (6 2 1% ) ,不符合或有新发现 11例 (37 9% )。 2 0例不孕症患者 ,发现子宫内膜异位病灶 9例 (45 % ) ;18例行输卵管美蓝通液术。腹腔镜监视下子宫纵隔电切割术 6例及子宫粘膜下肌瘤剜出术 2例。 2 9例术后恢复良好 ,均未出现并发症。　结论　宫、腹腔镜联合检查与手术适用于怀疑宫腔、腹腔同时存在病变的病例 ;对于子宫穿孔可能性较大的宫腔镜手术 ,应采用腹腔镜监视 ;在不孕症的检查与治疗中具有重要价值

Combined Hysteroscopy and Laparoscopy in Gynecological Examination and Operation (A report of 29 cases)

Objective To study the indication and advantages of combined hysteroscopy and laparoscopy in gynecological examination and operation. Methods We conducted a retrospective study of 29 cases with combined hysteroscopy and laparoscopy in gynecological examination and operation. Results Among the 29 cases, 18 cases (62.1%) showed examination results in accordance with pre-operation diagnoses, and 11 cases (37.9%) had new or inconsistent results. Among 20 cases with infertility, endometriosis was found in 9; 18 cases received the fallopian tube methyleneblue smoothness test. Under laparoscopic monitoring, 6 cases received hysteroscopic electric resection and 2 cases submucous myomaectomy. All the 29 cases recovered well and had no complication. Conclusion Examinations and operations using combined hysteroscopoy and laparoscopy apply to the cases which are suspicious of having pathological changes both in the uterine cavity and abdominal cavity. Hysteroscopy should be performed under laparoscopic monitoring in the cases with probable danger of uterus perforation. They are of great value in the examination and treatment of infertility.
