Abstract:Objective To investigate the predictive value of fibrinogen combined with D-dimer on the clinical prognosis of COVID-19.Methods The clinical data of 281 hospitalized COVID-19 patients were retrospectively analyzed to explore the correlation between fibrinogen combined with D-dimer and the prognosis of COVID-19.Results A total of 281 hospitalized COVID-19 patients were collected,including 157 males and 124 females,aged 24-97 years,with an average of 56.7±13.7 years.Of the 281 patients,80 were normal(28.5%),132 were severe(47.0%),and 69 were critical(24.6%).A total of 171 cases(61.2%)were complicated with at least one chronic disease.The patients in the death group were older than the surviving group,and the proportion of elderly patients with a history of underlying disease was also higher than that in the surviving group(P<0.05).The hemoglobin and lymphocyte counts in the death group were lower than those in the surviving group(P<0.0001).The white blood cell count,monocyte count,and C-reactive protein levels in the death group were higher than those in the survival group(P<0.05),and the plasma albumin level in the death group was lower than those in the survival group(P<0.0001).The indexes of myocardial injury(TnI,BNP)in the death group were higher than those in the survival group(P<0.01),and the indexes of fibrinolysis(D-dimer,fibrinogen)in the death group were higher than those in the survival group(P<0.0001).Cox regression suggested that lymphopenia,hypoproteinemia,myocardial injury,and abnormal fibrinolysis were risk factors associated with death during hospitalization.Elevated D-dimer(≥2.00 mg/L)combined with increased fibrinogen(≥8.11 g/L)had a sensitivity 0.80 and specificity 0.81 in COVID-19 patients during hospitalization.Conclusion D-dimer combined with fibrinogen are predictors for death of COVID-19.Early anti-inflammatory combined with anticoagulation therapy may be a way to improve the success rate of COVID-19 clinical treatment.
