Risk factors of multiple bronchoscopic lavage therapy in children with refractory lobar pneumonia

Objective To investigate the risk factors of single and multiple bronchoscopic lavage therapy in children with lobar pneumonia.Methods The clinical data of 122 lobar pneumonia children at Department II of Respiratory Disease,Xi′an Children′s Hospital from January 2017 to August 2019 were retrospectively.The patients were divided into the single group and the multiple group(≥2 times),and the correlative index were compared between the two groups.Logistic regression analysis was performed to analyze the risk factors of multiple bronchoscopic lavage therapy in lobar pneumonia patients.Results Among 122 children,67 cases were in the single group and 55 cases in the multiple group.Children undergoing multiple bronchoscopy having the fever duration≥10 days before the bronchoscopy were more in the multiple group than in the single group(P<0.05).At the same time,the occurrence rate of pleural effusion and pulmonary atelectasis of the multiple group were higher than those of the single group(P<0.05).In addition,the rate of the total of CRP≥10 mg/L was higher in the multiple group than in the single group(P<0.05).Under bronchoscopy,the occurrence of mucous plug was higher in the multiple group than in the single group(P<0.05).After adjusted by multiple regression analysis,it found five factors were independent risk factors for multiple bronchoscopic procedures,which were fever duration before bronchoscopy,pleural effusion,pulmonary atelectasis,and CRP≥10mg/L.Conclusion Whether or not to actively begin bronchoscopy in early stage,inflammatory reaction,occurrence of the complication and the mucous plug in bronchoscopy are important risk factors for multiple bronchoscopic procedures.
