Application of lung ultrasound score in the diagnosis and prognosis evaluation of neonatal respiratory distress syndrome

Objective To analyze the predictive role of lung ultrasound score in the diagnosis and prognosis of neonatal respiratory distress syndrome(NRDS).Methods 145 cases of NRDS were selected as the NRDS group.In addition,150 neonates born without respiratory diseases during the same period were selected as the control group.NRDS children were divided into the good prognosis group and the poor prognosis group according to their clinical outcomes at discharge.The pulmonary ultrasound scores and other factors that may affect the prognosis were compared between the two groups.Logistic regression analysis was used to identify the influencing factors of poor prognosis in children with NRDS.Receiver operating characteristic curve(ROC)was drawn to analyze the predictive value of pulmonary ultrasound score in the death of children with NRDS.Results The lung ultrasound score of the NRDS group was lower than that of the control group(P<0.05).The gestational age and pulmonary ultrasound score of the poor prognosis group were lower than those of the good prognosis group,the proportion of gestational diabetes mellitus and white blood cell count(WBC)were higher than those of the good prognosis group,and the first use time of PS and prothrombin time(PT)were longer than those of the control group(P<0.05).Logistic regression analysis showed that gestational age,gestational diabetes mellitus,time of first use of PS,PT,white blood cell count and pulmonary ultrasound score were independent influencing factors of poor prognosis in children with NRDS(P<0.05).ROC analysis showed that the sensitivity,specificity,accuracy and AUC were 89.47%,80.37%,82.76%and 0.821,respectively.Conclusion The pulmonary ultrasound score of children with NRDS is lower than that of newborns without respiratory diseases,and it is an independent influencing factor of poor prognosis of NRDS,which can be used for monitoring the disease and judging the prognosis of NRDS.
