Clinical characteristics of the neurological damage caused by the abuse of nitrous oxide (report of one case)

Objective To investigate the clinical characteristics of the neurological damage caused by the abuse of nitrous oxide. Methods The clinical data of 2 patients with neurological impairment caused by the abuse of nitrous oxide were retrospectively analyzed, and the relevant literatures were reviewed. Results One case mimics the symptoms of Guillain-Barré syndrome, manifested as symmetrical numbness and weakness. T2WI of the spine showed the "V" signal in the posterior cervical cord, and the EMG showed multiple peripheral nerve demyelination and axonal injury. The other case mimics the symptoms of acute myelitis, manifested as weakness of both lower limbs with urinary retention, EMG showed spinal cord damage, while no obvious imaging lesions were found on spinal cord. Blood test was characterized by high homocysteine levels, with or without vitamin B12 deficiency. After a cessation of contact with nitrous oxide and administration of vitamin B12,patients gradually returned to normal. Conclusion Healthy young people present with neurological symptoms of vitamin B12 deficiency, such as subacute combined degeneration, peripheral neuropathy, etc., the possibility of nitrous oxide poisoning should be considered.
