简易呼吸训练器在AECOPD康复治疗中的疗效观察

目的观察简易呼吸训练器在慢性阻塞性肺疾病(简称慢阻肺)急性加重期呼吸康复治疗中的疗效,探讨其在呼吸康复治疗中的可行性。方法选取2018年1月-2018年12月在福建医科大学附属协和医院呼吸科门诊及住院的慢阻肺急性加重期患者60例,分为治疗组和对照组。治疗组在镇咳、祛痰、抗感染、支气管扩张剂、糖皮质激素、吸入剂、雾化、氧疗等常规治疗的前提下给予简易呼吸训练器进行呼吸功能锻炼,两组的治疗前后评估第一秒用力呼气流量(FEV₁%)、6分钟步行试验(6MWT)、CAT评分(COPD assessment test)和改良英国MRC呼吸困难指数(mMRC评分)。结果治疗后两组患者6MWT、CAT及mMRC评分指标均较治疗前有改善(P<0.05),治疗后治疗组6MWT、CAT评分的改善程度优于同期对照组水平(P<0.05)。治疗后两组患者FEV 1%值与治疗前相比,差异无统计学意义(P>0.05),与同期对照组组间比较无统计学意义(P>0.05)。患者对呼吸锻炼的满意度评分为90.45±7.08分。结论采用简易呼吸训练器对慢阻肺急性加重期患者进行呼吸功能锻炼是安全有效、满意度高,适用于临床推广、应用。

Efficacy of rehabilitation therapy in patients with AECOPD with simple breathing apparatus

Objective To observe the curative effect of simple breathing apparatus in patients with AECOPD.Methods The clinical data of 60 cases with AECOPD in Fujian Medical University Union Hospital from January 2018 to December 2018 were selected and divided into two groups(the treatment group and the control group).On the basis of routine medical treatment,including antitussive,anti-infection,bronchodilator,glucocorticoid,inhalation,atomization and oxygen therapy,the treatment group was given pulmonary rehabilitation using simple breathing apparatus.FEV₁%,6MWT,CAT and mMRC pre and post pulmonary rehabilitation were evaluated.Results After treatment,the value of 6MWT,CAT and mMRC scores in both groups were improved than before(P<0.05),and the improvement of 6MWT and CAT was more pronounced in the treatment group than in the control group(P<0.05).The FEV 1%value in both groups showed no significantly improvement after treatment than before(P>0.05),and there was no significant difference between the two groups during the same period(P>0.05).The satisfaction score of pulmonary rehabilitation was 90.45±7.08.Conclusion It is safe and effective for patients with AECOPD to perform pulmonary rehabilitation with simple breathing apparatus.Not only the patients are highly satisfied with the apparatus,but also it is suitable for clinical application.
