摘    要:目的比较3D打印模型桥接组合内固定系统和外侧解剖锁定钢板内固定系统治疗胫骨平台骨折的效果。方法将94例胫骨平台骨折患者按治疗方法的不同分为观察组(采用3D打印模型桥接组合内固定系统治疗,51例)和对照组(采用外侧解剖锁定钢板内固定系统治疗,43例)。比较两组手术情况和并发症发生情况。术后1年采用HSS评分标准评价膝关节功能优良率。结果患者均获得随访,时间1~3年。手术时间、术中出血量、住院时间、骨折愈合时间观察组均明显短(少)于对照组(P<0.05)。解剖复位率观察组略高于对照组,但差异无统计学意义(P>0.05)。术后1年HSS评分优良率两组比较差异无统计学意义(P>0.05)。并发症发生率观察组明显低于对照组(P<0.05)。结论与外侧解剖锁定钢板内固定系统相比,3D打印模型桥接组合内固定系统治疗胫骨平台骨折具有缩短手术时间、减少出血量、降低并发症发生率的优势,临床疗效满意。

Abstract:Objective To compare the effects of 3D printing model assisting bridge-link combined internal fixation system and lateral anatomical locking plate internal fixation system in the treatment of tibial plateau fractures.Methods Ninety-four patients with tibial plateau fractures were divided into the observation group(51 cases were treated with 3D printing model assisting bridge-link combined internal fixation system)and the control group(43 cases were treated with anatomical locking plate internal fixation system).The operation situation and complications were compared between two groups.At 1 year postoperation,the excellence-good rate of knee function were evaluated with HSS score system.Results All patients were followed up for 1~3 years.The operation time,intraoperative bleeding volume,hospitalization time and fracture healing time in the observation group were significantly shorter(less)than those of the control group(P<0.05).The anatomical reduction rate of the observation group was slightly higher than the control group,while there was no statistical significance between two groups(P>0.05).At one year postoperation,there was no significant difference in the excellent-good rate of HSS score between the two groups(P>0.05).The complication incidence rate in the observation group was significantly lower than that in the control group(P<0.05).Conclusions Compared with the lateral anatomical locking plate internal fixation system,3D printing model assisting bridge-link combined internal fixation system has advantages of shortening the operation time,reducing the bleeding volume and the incidence of complications,which can obtain satisfactory clinical effect.
