Evaluation of left ventricular systolic function in patients with hyperuricemia by three-dimensional speckle tracking imaging

Objective To evaluate the left ventricular systolic function in patients with hyperuricemia(HU)by threedimensional speckle tracking imaging(3 D-STI).Methods Twenty-five hyperuricemia patients(HU group)and 30 healthy volunteers(control group)matched by sex and age were selected.Left atrial end systolic diameter(LAD),left ventricular end diastolic diameter(LVEDd),and left ventricular end systolic diameter(LVESD)were measured by conventional two-dimensional echocardiography,and left ventricular end diastolic volume(LVEDV),left ventricular end systolic volume(LVESV)and left ventricular ejection fraction(LVEF)were measured by Simpson biplane method.At the same time,3 D-STI was applied to measure the global longitudinal strain(GLS),global radial strain(GRS),global circumferential strain(GCS)and global area strain(GAS)of the left ventricle,and carry on the correlation analysis with blood uric acid value.The ROC curve was drawn to analyze the diagnostic efficacy of left ventricular systolic function damage predicted by 3 D-STI parameters in patients with HU.Results Compared with the control group,LAD,LVEDD,LVESD,LVEDV,LVESV and LVEF in HU group had no significant difference.The left ventricular GLS,GCS and GAS in HU group were reduced significantly,and the difference were statistically significant(all P<0.05).Correlation analysis showed that blood uric acid value was negatively correlated with GLS and GCS(r=-0.571,-0.463,both P<0.05).ROC curve showed that among the 3 D-STI parameters,when the cut-off value of GLS was20.35%,the area under the curve of GLS was 0.919,the sensitivity was 95%,the specificity was 66%,and Youden index was0.61.Conclusion 3 D-STI can evaluate the changes of left ventricular systolic function in patients with HU in early stage.
