骶尾旁入路治疗成人骶前囊肿

目的探讨成人骶前囊肿的临床特点及经骶尾旁入路治疗效果。方法回顾性分析2000年至2004年之间诊治的23例成人骶前囊肿的临床特点及治疗方法并结合文献分析。结果23例均表现为不同程度的骶尾或会阴部疼痛伴肛门坠胀,直肠腔内B超和MRI检查准确率100%。手术采用经骶尾旁入路,21例囊肿完全切除,2例部分切除。无直肠损伤及相关手术并发症。结论骶前囊肿常无典型临床表现,直肠腔内B超可作为初步诊断的首选检查。经骶尾旁入路可以治疗大多数骶前囊肿,手术创伤小,出血少,明显降低了相关并发症。

Parasacrococcygeal approach for the management of adult presacral cyst

Objective To investigate the clinical characteristics and parasacrococcygeal approach to the management of presacral cyst.Methods Clinical data of 23 cases of adult presacral cyst(from 2000 to 2004) were analyzed retrospectively and reviewed.Results All patients presented with sacrococcygeal or perineal pain,and/or rectal fullness.Both Transrectal ultrasound and MRI had an accuracy of 100%.All patients were treated with parasacrococcygeal approach.There were 21 cases of completely resectable lesions and 2 cases of incompletely resection.There were no rectal injuries and relative complications.Conclusion Presacral cyst are often asymptomatic.Transrectal ultrasound can be selected as primary examination for clinical diagnoses.Parasacrococcygeal approach can be used to manage majority of presacral cysts with less blood loss and lower complication rate.
