| 摘 要： | 目的：探讨多层螺旋CT血管成像（MSCTPA）技术在肺动脉栓塞（PE）中的应用价值。方法：收集16排螺旋CT诊断肺动脉栓塞患者18例，图像后处理采用最大密度投影（MIP）、多平面重组（MPR）、容积再现（VR）等方法显示肺动脉并分析其表现。结果：18例PE患者中，左右肺动脉栓塞9支，叶肺动脉栓塞27支，段肺动脉栓塞98支，亚段肺动脉栓塞64支。PE的直接征象为肺动脉充盈缺损，其中，中心型缺损46支，可见“悬浮征”或“轨道征”，环壁缺损型42支，附壁缺损型74支，完全性阻塞36支。PE的间接征象有肺梗死灶4例，局限性肺纹理稀疏12例，马赛克征6例，肺动脉高压8例，两侧胸腔积液3例，心包少量积液2例。结论：MSCTPA可以清晰显示肺动脉的栓塞部位和血栓栓子的形态，应该作为确诊PE的主要检查方法。
