可逆性开放血脑屏障在脑胶质瘤动脉化疗中的应用

目的：探讨可逆性开放血脑屏障在动脉介入治疗脑胶质瘤中的应用价值。方法：在SD大鼠脑尾状核头部种植C6大鼠胶质瘤细胞后，在加或不加20％甘露醇（5ml)的情况下，对荷瘤大鼠进行动脉介入化疗，并与静脉化疗对比，化疗药物为替尼泊苷（VM－26）和甲氨蝶呤（MTX）。使用超导磁共振成像系统检查肿瘤种植后14d及化疗后20d的肿瘤大小，以判断疗效。结果：VM－26动脉化疗组在使用和不使用甘露醇之间无显性差异。MTX动脉化疗组在使用和不使用甘露醇之间疗效有显性差异（P＜0．05）。VM－26与MTX的甘露醇动脉化疗组之间疗效无显性差异。单纯VM－26动脉化疗组疗效明显优于静脉化疗组。结论：甘露醇对脂溶性化疗物疗物疗效的影响不大；但大用于溶性化疗物药进行化疗前，应用开放血脑屏障的技术有利于增其疗效。

Reversible opening blood brain barrier with mannitol in intra-arterial chemotherapy of glioma

Objective: To evaluate the value of the disruption of blood brain barrier(BBB) with mannitol to the intra arterial chemotherapy of glioma. Methods: Under stereotactic,C6 rat glioma cells were implanted into the right caudate nucleus of male SD rats.Superconduct MRI system,Gd DTPA as MRI contract,were used to measure rat brain tumor volume.Two chemotherapy drugs, teniposide (VM 26) and methotrexate (MTX),were used in intra arterial and venous chemotherapy. Results: There was no significant statistical difference between VM 26 with or without mannitol.There had significant statistical difference between MTX with or without mannitol.There was no statistical difference between VM 26 with or without mannitol and MTX with mannitol.Conclusion: There are no certain significance that mannitol affect the curative effect in treating brain tumor with lipophilic anti brain tumor compounds.It is necessary to open BBB before water soluble compounds of anti brain tumor are used.
