糖尿病患者脉压与冠脉病变相关性研究

摘    要:目的研究糖尿病患者24h平均脉压与冠心病之间的关系,探讨冠心病新的危险因素。方法选择174例糖尿病患者,均进行24h动态血压检测(ABPM)及选择性冠状动脉造影术(SCA),根据SCA结果分为冠心病组121例(A组)及非冠心病组53例(B组),对比2组患者的临床资料,比较2组患者24h平均脉压(PP);根据脉压结果分为PP〉40mmHg组128例及PP≤40mmHg组46例,比较2组合并冠心病比率。结果PP:A组(60.1±9.3)mmHg,大于B组(26.5±10.2)mmHg,差异有统计学意义(P〈0.01);PP〉40mmHg组有105例冠心病患者(82.0%),PP≤40mmHg组有冠心病患者16例(34.8%),差异有统计学意义(P〈0.05)。结论糖尿病患者脉压增大可预测冠心病的发生。

Abstract:Objective To study the relations between 24 hours average pulse pressure and coronary heart disease in diabetes patients, and to discuss new risk factors for coronary heart disease (CHD). Methods Selecting 174 cases of diabetic patients, 102 cases of male and 72 female. All patients have been operated with 24h ambulatory blood pressure monitor (AB- PM) and selective coronary angiography (SCA). In accordance with the results of SCA, divided into 2 groups, i.e. the CHD group (group A) and non-CHD group (group B). Analyzing two groups of patients with clinical data, contrast two groups of patients with 24h average pulse pressure (PP) , by the results of PP value larger or less than 40mm Hg, the patients could be redivided into 2 groups. Results The clinical data of the two groups of patients is of comparability; PP of group A is ( 60. 1 ±9.3 )mm H g, significantly greater than PP of group B ( 26.5 ± 10.2 ) mm Hg ( P 〈 0.01 ) ; Totally there are 128 patients with PP 〉40mm Hg, of which 105 cases of patients with CHD (proportionally 82.0% ), and 46 patients with PP≤ 40mm Hg, of which 16 cases of patients with CHD (34.8%), the difference was significant(P 〈 O. 05). Conclusion Pulse pressure is a risk factor for CHD. Increased PP in diabetes patients can predict the occurrence of CHD.
