针刺联合中药离子导入治疗干眼症临床研究

目的探讨针刺联合中药离子导入治疗干眼症的临床疗效及对眼表功能、泪膜的影响。方法采用随机数字表法将120例干眼症患者分为观察组和对照组各60例。对照组予玻璃酸钠滴眼液,1滴/次,5~6次/d,6 d为1个疗程;观察组予针刺联合中药离子导入,隔日1次,3次为1个疗程。2组均连续治疗4个疗程。观察2组临床疗效,比较2组治疗前后眼表疾病指数量表(OSDI)评分、临床症状评分及基础泪液分泌试验(SIT)、泪膜破裂时间(BUT)、角膜荧光素染色(FL)评分。结果观察组总有效率为83.33%(50/60),对照组为61.67%(37/60),2组比较差异有统计学意义(P<0.05)。与本组治疗前比较,2组治疗后OSDI评分明显降低(P<0.05);2组治疗后比较,观察组OSDI评分明显低于对照组(P<0.05)。与本组治疗前比较,2组治疗后眼部干涩、畏光、异物感、视疲劳、视物模糊评分明显降低(P<0.05);2组治疗后比较,观察组上述临床症状评分明显低于对照组(P<0.05)。与本组治疗前比较,2组治疗后SIT增加,BUT延长,FL评分降低,差异有统计学意义(P<0.05);2组治疗后比较,观察组SIT、BUT、FL改善情况明显优于对照组(P<0.05)。结论针刺联合中药离子导入治疗干眼症临床疗效显著,可改善眼表功能,促进泪液分泌,提高泪膜稳定性。

Clinical Study on Acupuncture Combined with TCM Iontophoresis for Treatment of Xerophthalmia

Objective To explore the clinical efficacy of acupuncture combined with TCM iontophoresis for xerophthalmia and its effects on eye surface function and tear film.Methods Totally 120 patients with xerophthalmia were divided into observation group(60 cases)and control group(60 cases)according to random number table method.The control group was treated with sodium hyaluronate eye drops,one drop per time,5 to 6 times per day,6 d as a treatment course,and the observation group was treated with acupuncture combined with TCM iontophoresis,once every other day,three times as a treatment course.Both groups received consecutive four treatment courses.The clinical efficacy of the two groups was compared,and the ocular surface disease index scale(OSDI)score,clinical symptom score and tear secretion(SIT),tear break-up time(BUT),and corneal fluorescence staining(FL)were observed before and after treatment.Results The total effective rate in the observation group was 83.33%(50/60),and the control group was 61.67%(37/60),with statistical significance(P<0.05).Compared with before treatment,the OSDI score of the two groups decreased significantly after treatment(P<0.05);After treatment,the OSDI score in the observation group was significantly lower than that in the control group(P<0.05).Compared with before treatment,the dryness,photophobia,foreign body sensation,visual fatigue,and blurred vision scores of the two groups were significantly reduced after treatment(P<0.05);After treatment,the scores of the above symptoms in the observation group were significantly lower than those in the control group(P<0.05).Compared with before treatment,the SIT increased,BUT prolonged,and FL score decreased after treatment in the two groups,with statistical significance(P<0.05);After treatment,the improvement of SIT,BUT,and FL in the observation group was significantly better than that in the control group(P<0.05).Conclusion Acupuncture combined with TCM iontophoresis is effective in the treatment of xerophthalmia,which can improve the function of eye surface,promote the secretion of tear fluid and improve the stability of tear film.
