AMI患者血清miR-208、VEGF-B与左室重构及炎症反应的关系

摘    要:目的探讨急性心肌梗死(AMI)患者血清微小RNA-208(miR-208)、血管内皮生长因子-B(VEGF-B)的变化与患者左室重构及炎症反应的关系。方法选取遂宁市中心医院2017年1月至2018年8月收治的90例AMI患者(AMI组)、健康对象90例作为对照组,检测对比两组的血清miR-208、VEGF-B、白细胞介素-10(IL-10)、肿瘤坏死因子-α(TNF-α)、可溶性细胞间黏附分子(sICAM-1)水平,采用超声心动图检测左室舒张末期容积(LVEDV)、左室舒张末期内经(LVEDD)、左室射血分数(LVEF)。结果 AMI组患者的血清miR-208水平显著的高于对照组,差异具有统计学意义(t=16.445,P <0.05),AMI组患者的血清VEGF-B水平显著的低于对照组,差异具有统计学意义(t=-11.625,P <0.05);AMI组患者的血清IL-10、TNF-α、sICAM-1水平显著的高于对照组,差异具有统计学意义(t=19.236、17.910、36.624,P <0.05);治疗12个月后,AMI组患者的LVEDV、LVEDD均显著的高于入院时(t=-7.697、-9.602,P <0.05),治疗12个月后的LVEF与入院时比较差异无统计学意义(t=1.911,P> 0.05);AMI患者的血清miR-208水平与LVEDV、LVEDD均呈显著的正相关关系(r=0.594、0.621,P <0.05),AMI患者的血清VEGF-B水平与LVEDV、LVEDD均呈显著的负相关关系(r=-0.552、-0.497,P <0.05);AMI患者的血清miR-208水平与血清IL-10、TNF-α、sICAM-1水平均呈显著的正相关关系(r=0.591、0.642、0.618,P <0.05),AMI患者的血清VEGF-B水平与血清IL-10、TNF-α、sICAM-1水平无明显的相关关系(r=-0.095、-0.117、0.153,P> 0.05)。结论 AMI患者早期出现血清miR-208表达升高、VEGF-B水平降低,可能与AMI患者炎症反应程度、远期心室重构形成有关。

Abstract:Objective To investigate the relationship between serum microRNA-208(miR-208), vascular endothelial growth factor-B(VEGF-B) and left ventricular remodeling and inflammatory response in patients with acute myocardial infarction(AMI). Methods 90 patients with AMI(AMI group) and 90 healthy subjects were enrolled in Suining central hospital from January 2017 to August 2018. The serum levels of miR-208, VEGF-B and interleukin-10(IL-10), tumor necrosis factor-α(TNF-α), soluble intercellular adhesion molecule(sICAM-1) levels were compared between the two groups. Left ventricular end-diastolic volume(LVEDV), left ventricular end-diastolic period(LVEDD), left ventricular ejection fraction(LVEF) were detected by echocardiograph. Results The serum miR-208 level in the AMI group was significantly higher than that in the control group(t=16.445, P < 0.05). The serum VEGF-B level in the AMI group was significantly lower than that in the control group, and the difference was statistically significant(t=-11.625, P < 0.05);serum IL-10, TNF-α, sICAM-1 levels in the AMI group were significantly higher than the control group, the differences were statistically significant(t=19.236, 17.910, 36.624, P < 0.05);after 12 months of treatment, LVEDV,LVEDD in the AMI group were significantly higher than those at admission(t=-7.697,-9.602, P < 0.05). There was no significant difference in LVEF after 12 months of treatment and at admission(t=1.911, P > 0.05). Serum miR-208 levels has a significant positive correlation with LVEDV and LVEDD in AMI patients(r=0.594, 0.621, P < 0.05). Serum VEGF-B level was significantly negatively correlated with LVEDV and LVEDD in AMI patients(r=-0.552,-0.497, P < 0.05). There were significant positive correlations between the levels of miR-208 and IL-10、TNF-α、sICAM-1(r=0.591, 0.642, 0.618, P < 0.05). There were no significant correlation between serum VEGF-B levels and serum IL-10, TNF-α and sICAM-1 levels in AMI patients(r=-0.095,-0.117, 0.153, P > 0.05). Conclusion In the early stage of AMI, the expression of serum miR-208 elevated,the level of VEGF-B decreased.This may be associated with inflammatory response and long-term ventricular remodeling in patients with AMI.
