阻塞性睡眠呼吸暂停综低通气合征合并高血压患者的临床特点分析

目的 :探讨阻塞性睡眠呼吸暂停低通气综合征合并高血压患者的临床特点。方法选取2018年1月~2019年6月就诊于武汉大学人民医院耳鼻喉咽喉头颈外科,行多导睡眠呼吸监测(PSG)的682例成人进行回顾性分析。结果 OSAHS患者中合并高血压者约为46.9%;单纯OSAHS组与OSAHS合并高血压组性别分布、烟酒史等既往病史、血糖、血脂等指标差异无统计学意义(P>0.05);与单纯OSAHS患者相比,OSAHS合并高血压患者的OSAHS严重程度及低血氧症程度分布发生变化,这种变化不受高血压分级的影响;两组患者在AHI、LSaO2、MSaO2、氧减指数等指标的差异具有统计学意义(P<0.05),这种差异在男性患者中更为明显;单纯OSAHS组与OSAHS合并高血压组患者主观嗜睡评分无明显差别(P>0.05),男性患者中两组ESS评分也无明显差别(P>0.05),而女性OSAHS合并高血压组ESS评分明显低于单纯OSAHS组(P<0.05)。结论 OSAHS合并高血压患者的AHI及血氧指标较单纯OSAHS患者更为严重,这种改变存在性别差异,在男性患者中更为明显。

Analysis on the clinical characteristics in patients of Obstructive sleep apnea hypopnea syndrome with hypertension

Objective To explore the clinical characteristics of obstructive sleep apnea hypopnea syndrom(OSAHS)with hypertension.Methods A total of 682 adult patients who had done examination of polysomnogtaphy(PSG)in otorhinolaryngology clinic of Renmin Hospital of Wuhan University between January 2018 to June 2019 were analyzed retrospectively.Results The incidence of hypertension in OSAHS patients was about 46.9%.There was no significant difference in gender distribution,smoking and alcohol history,blood glucose,blood lipid level or other indicators between the simple OSAHS group and OSAHS with hypertension group(P>0.05).Compared with simple OSAHS group,the distribution of OSAHS severity and hypoxemia degree in patients of OSAHS combined with hypertension group had changed,not affected by the grading of hypertension.The differences of AHI,LSaO2,MSaO2,oxygen decreasing index and some other indicators between two groups were statistically significant(P<0.05),which was only reflected in male patients.There was no significant difference in the score of Epworth sleepiness scale(ESS)between the simple OSAHS group and OSAHS with hypertension group(P>0.05).And there was no significant difference in the ESS score between the two groups of male patients(P<0.05),while the ESS of OSAHS with hypertension group was significantly lower than the simple OSAHS group in female patients(P<0.05).Conclusions Compared with the simple OSAHS group,the parameters of respiratory disturbance and blood oxygen level in patients of OSAHS with hypertension are more serious,especially in male patients.
