宫颈癌彩色多普勒超声血流信号与微血管密度关系对预后的评估价值

目的探讨彩色多普勒超声血流信号与微血管密度(MVD)的关系对宫颈癌患者预后的评估价值。方法选取我院收治的宫颈癌患者100例为观察组和宫颈防癌检查的健康组女性50例,分析观察组患者的资料及1年的跟踪随访情况,以及健康组的检查结果。对150例观察对象行彩色多普勒超声血流检查,并采用免疫组化计数测量微血管密度,分析宫颈癌患者血流阻力指数(RI)和微血管密度(MVD)的关系。结果观察组患者的RI、MVD分别低于和高于健康组(P<0.01),宫颈癌患者RI与MVD呈负相关(r=-0.496,P<0.001);多因素Logistic回归分析显示,MVD为宫颈癌患者预后情况(差)的影响因素(95%CI1.01~1.21,P<0.05),ROC曲线显示,MVD对患者的预后有一定的预测价值,AUC为0.758,灵敏度为97.3%、特异度44.8%。结论宫颈癌患者彩色多普勒血流阻力值和MVD为负相关关系,MVD对患者的预后有一定的预测价值。

The clinical value of blood flow signal and microvessel density in the prognosis of cervical cancer by color Doppler ultrasound

Objective To investigate the relationship between color Doppler ultrasound blood flow signal and MVD in patients with cervical cancer and to analyze the value of color Doppler ultrasound in evaluating the prognosis of patients with cervical cancer.Methods From January 2017 to December 2018,100 patients with cervical cancer in our hospital were selected as 50 healthy women in the observation group and cervical cancer prevention examination.The case data of the observation group,the follow-up of one year and the results of the health group were reviewed and analyzed.Color Doppler ultrasound was performed in 150 subjects,and tumor microvessel density was measured by immunohistochemistry.The relationship between blood flow resistance index(RI)and microvessel density(MVD)in patients with cervical cancer was analyzed,and the prognostic value of RI and MVD in patients with cervical cancer was discussed.Results The RI and MVD in the observation group were lower than those in the healthy group(P<0.01).RI was negatively correlated with MVD in patients with cervical cancer(r=-0.496,P<0.001).Multivariate logistic regression analysis showed that MVD was the influencing factor of prognosis of cervical cancer patients(95%CI1.01~1.21,P<0.05).ROC curve showed that MVD had predictive value for prognosis,AUC 0.758,sensitivity of 97.3%and specificity of 44.8%.Conclusion The color Doppler flow resistance of cervical cancer patients and MVD were negatively correlated,and MVD had certain predictive value for prognosis.
