卵巢颗粒细胞瘤的临床特点和CT诊断分析

目的分析卵巢颗粒细胞瘤的临床资料及CT资料,提高对该疾病的诊断能力。方法回顾性分析13例经手术病理证实的卵巢颗粒细胞瘤的临床及CT资料。结果13例患者中11例初发,且均为单个病灶,左侧卵巢5例,右侧卵巢6例,呈圆形或椭圆形。2例患者为复发,1例单发病灶,呈不规则形,1例3个病灶,呈圆形或椭圆形。CT表现初发病例与复发病例相似,多数肿块边缘光滑,与周围组织分界清楚,11个病灶表现为囊实性肿块,2个病灶以实性为主肿块,2个病灶以囊性为主肿块。增强扫描实性部分、囊壁及分隔不同程度强化,3例呈轻度强化,8例呈中度强化,2例呈明显强化。8例患者伴有不同程度子宫体积增大、子宫内膜增厚,3例患者伴有子宫肌瘤,6例患者伴有腹水。结论卵巢颗粒细胞瘤的临床特点及CT表现具有一定的特征性,但缺乏特异性,综合分析有助于提高该疾病的正确诊断率。

Clinical characteristics and CT diagnosis of ovarian granulosa cell tumor

Objective To analyze the clinical and CT data of OGCT and to promote the diagnostic ability of the disease.Methods The clinical and CT data of 13 patients with OGCT confirmed by histopathology were retrospectively analyzed.Results Among the 13 patients,11 cases were primary tumor and all cases were single lesions,including 5 cases in the left ovary and 6 cases in the right ovary,which were round or oval.2 cases were recurrent,1 case was single lesion with irregular shape and 1 case consisted of 3 lesions with round or oval shape.CT manifestations of primary cases were similar to recurrent cases.Most of the masses had smooth edges and clear boundaries with surrounding tissues,11 lesions showed cystic-solid mass,2 lesions were mainly solid mass and 2 lesions were mainly cystic mass.The solid part,cyst wall and septum were enhanced in different degrees on enhanced scan.3 cases showed slight enhancement,8 cases showed moderate enhancement,and 2 cases showed obvious enhancement.The tumors were associated with enlarged uterus and endometrium thickening(n=8),uterine fibroids(n=3),ascites(n=6).Conclusion The clinical characteristics and CT manifestations of OGCT have certain characteristics but lack of specificity.Comprehensive analysis is helpful to improve the correct diagnosis rate of the disease.
