摘    要:目的:研究同型半胱氨酸(Hcy)对高血压患者心肌重构的影响。方法:根据血浆Hcy水平,2017年1月~2017年12月于我院治疗的92例高血压患者被分为H型高血压组(46例)和非H型高血压组(46例)。测量比较两组24h平均收缩压(24h mSBP)、24h平均舒张压(24h mDBP)、左房内径(LAD)、左室后壁厚度(LVPWT)、室间隔厚度(IVST)、左室舒张末期内径(LVEDd)及LVEF。结果:两组24h mSBP、24h mDBP、LAD、LVEDd、LVEF比较均无显著差异(P均>0.05)。与非H型高血压组比较,H型高血压组LVPWT9.80(9.38,10.73)mm比13.40(12.40,14.30)mm]、IVST9.70(9.23,10.63)mm比13.10(12.40,14.20)mm]均显著升高(P均=0.001)。结论:与非H型高血压者比较,H型高血压患者左室后壁厚度、室间隔厚度显著增厚,可能高血浆Hcy水平加剧了高血压患者心肌重构的进展。

Abstract:Objective:To study the influence of homocysteine(Hcy)on myocardial remodeling in patients with hypertension.Methods:According to plasma Hcy level,a total of 92 hypertensive patients treated in our hospital from Jan 2017 to Dec 2017 were divided into H-type hypertension group(n=46)and non-H-type hypertension group(n=46).The 24h mean systolic blood pressure(24h mSBP),24h mean diastolic blood pressure(24h mDBP),left atrial diameter(LAD),left ventricular posterior wall thickness(LVPWT),interventricular septal thickness(IVST),left ventricular end-diastolic dimension(LVEDd)and LVEF were measured and compared between two groups.Results:There were no significant difference in 24h mSBP,24h mDBP,LAD,LVEDd and LVEF between two groups(P>0.05 all).Compared with non-H-type hypertension group,there were significant rise in LVPWT9.80(9.38,10.73)mm vs.13.40(12.40,14.30)mm]、IVST9.70(9.23,10.63)mm vs.13.10(12.40,14.20)mm]in H-type hypertension group(P=0.001 both).Conclusion:Compared with non-H-type hypertension patients,the LVPWT and IVST significantly thickened in patients with H-type hypertension,so high plasma Hcy level contributes to accelerating the process of myocardial remodeling in patients with hypertension.
