超声在定量分析女性子宫脱垂中的应用

摘    要:目的经会阴二维超声对女性盆底进行超声检查,探讨超声在定量评估女性子宫脱垂中的价值。方法由两位操作者分别测量477例女性最大Valsalva动作后宫颈最低点至阴道外口的距离,用组内相关系数分析同一操作者测量的重复性及不同操作者测量的一致性,运用线性回归分析评价超声测量结果与临床评价结果的相关性。分别应用t检验与χ^2检验评价年龄、体质量指数、性别、新生儿体重、生产方式、产次等影响因素。结果1)组内相关系数结果显示同一操作者测量的重复性及两个不同操作者之间一致性均非常好,同一操作者ICC=0.94,95%CI=0.93~0.95;不同操作者组间ICC=0.95,95%CI=0.94~0.95;2)运用线性回归分析超声测量距离与临床评估结果的相关性较好,两者呈正相关。超声测值与临床测值相关方程为y=1.56+0.8x;3)受检者年龄、体质量指数、新生儿性别影响因素在子宫脱垂组与无子宫脱垂组中无明显统计学差异。新生儿出生体重、生产方式及产次影响因素在子宫脱垂组与无子宫脱垂组中差异具有统计学意义(P<0.05)。结论运用实时二维超声技术检测女性的子宫脱垂在同一操作者的重复性及不同操作者间的一致性均非常好,且与临床评估具有较好的相关性。新生儿出生体重、生产方式及产次对产妇盆底结构的近期影响较明显。

Abstract:Objective A pelvic floor ultrasound examination was performed on women by perineal two-dimensional ultrasound to investigate the value of ultrasound evaluation in quantitative evaluation of female uterine prolapse.Methods The distance between the lowest point of the cervix and the external vaginal opening of the 477 women with maximum Valsalva motion was measured by two operators.The distance between the lowest point of the cervix and the external vaginal opening of the 477 women after the maximum Valsalva movement was measured by two operators.The intra-group correlation coefficient was used to study the repeatability of the same operator measurement and the consistency of different operator measurements.Linear regression analysis was used to evaluate the correlation between the ultrasonic measurement results and the clinical evaluation results.The t-test and chi-square test were used to evaluate the influencing factors such as age,body mass index,gender,newborn weight,production mode and birth order.Results 1)The correlation coefficient results in the group showed that the repeatability of the same operator measurement and the consistency between two different operators were very good,the same operator ICC=0.94,95%CI=0.93~0.95;between different operators ICC=0.95,95%CI=0.94~0.95;2)The correlation between the ultrasonic measurement distance and the clinical evaluation results,were positively correlated.The correlation between the ultrasonic measurement and the clinical measurement was y=1.56+0.8x;3)There were no significant differences in the age,body mass index and neonatal gender influencing factors between the uterine prolapse group and the non-uterine prolapse group.The factors affecting birth weight,production mode and birth weight of the newborn were statistically significant in the uterine prolapse group and the non-uterine prolapse group(P<0.05).Conclusion The use of real-time two-dimensional ultrasound technology to detect female uterine prolapse in the same operator repeatability and consistency between different operators are very good,and has a good correlation with clinical evaluation.The newborn's birth weight,production methods and birth times have a significant impact on the maternal pelvic floor structure.
