外伤性十二指肠破裂（附11例报告）

目的：探讨十二指肠破裂的诊断与治疗方法。方法：回顾性分析近6年收治的11例外伤性十二指肠破裂病人的临床资料。结果：术前确诊4例，术中确诊7例。11例中发生胰腺囊肿1例，吻合口梗阻1例，术后胃扩张1例，腹腔感染2例，十二指肠漏3例，11例全部治愈。结论：外伤性十二指肠破裂临床比较少见，术前诊断困难，术后并发症较多；及时而仔细的剖腹探查甚为重要；术式的选择因损伤的部位、时间、复合伤程度及全身情况而定；手术的关键是十二指肠减压和腹腔充分引流；术后给予营养支持。

TRAUMATIC RUPTURE OF DUODENUM: AN ANALYSIS OF 11 CASE

Objective ：To discuss the diagnosis and treatment of duodenal rupture. Methods：Clinical data of 11 cases of traumatic rupture of duodenum in last six years were retrospectively analyzed.Results： Four cases confirmed preoperation, seven cases confirmed during operation.Among the 11 cases there were one case of pancreatic cyst,one case of anastomotic obstruction, one case of acute stomach expansion,two case of abdominal infection,and three cases of duodenal leakage, and all cases were cured. Conclusion ：Traumatic rupture of the duodenumis relatively rare; the preoperative diagnosis is difficult; postoperative complications are common; timely and detailed laparotomy is important.The choice of operative method depends on the injury site, time the extent of injuries and the body conditions. The key is duodenal decompression and peritoneal drainage; after operation giving nutritional support.
