平阳霉素治疗血管瘤与脉管畸形严重不良反应的防治

目的：探讨平阳霉素（PYM）治疗血管瘤与脉管畸形的严重不良反应的防治措施。方法：用PYM治疗血管瘤与脉管畸形346例，在治疗前后作血常规及X线胸片检查，治疗期间观察并防治局部及全身反应。结果：发生不良反应25例。其中发热14例，局部溃疡7例，呕吐1例，过敏性休克3例。结论：PYM治疗血管瘤与脉管畸形可发生过敏性休克及肺纤维化等严重不良反应。注射PYM前应做好充分的抢救准备；治疗过程中控制好PYM的使用总量，注意随访胸部X线片，一旦出现肺纤维化，应立即停药。

Clinical analysis of adverse effects of pingyangmycin in treatment of hemangiomas and vascular mafformations

Objective：To evaluate the adverse effects of pingyangmycin （PYM） in treating hemangioma and vascular malformations and approach the methods of prevention and cure to the adverse effects,especially to the serious ones. Methods：The clinical data of 346 hemangiomas and vascular malformations treated with PYM were reviewed. To observe and cure the adverse effects during treatment period. Results：25 patients developed adverse effects.Among them, 14 had fever,7 had localized necrotic ulcer, 1 had vomitus and 3 developed allergic shock. Conclusion：Serious adverse effects,such as allergic shock and pulmonary fibrosis,would take place during and after treating hemangioma and vascular malformations by PYM. Sufficient messures of rescue should be prepared prior to use of PYM. The total amount of PYM should be controlled ,following-up X-ray examination of chest should be performed during treatment period.Drug should be withdrawn immediately once pulmonary fibrosis occurrs.
