二维及彩色多普勒超声在甲状腺肿块中的诊断价值

摘    要:目的:探讨二维及彩色多普勒超声在甲状腺肿块中的诊断价值。方法:对经手术病理证实的18例甲状腺腺瘤.37例结节性甲状腺肿及6例甲状腺癌的二维及彩色多普勒超声进行比较。结果:甲状腺腺瘤、结节性甲状腺肿及甲状腺癌在二维声像图。彩色血流分布,RI及Vmax方面差异有显著性(P〈0.05)。甲状腺腺瘤包膜、边缘晕环及环状血流;结节性甲状腺肿多发结节、结节周围组织回声改变以及甲状腺癌低回声及钙化在诊断中特异性较高。但多源性肿块的存在,如腺瘤、结节伴甲状腺癌缺乏上述特征,误诊率较高。结论:二维及彩色多普勒超声相结合有助于甲状腺肿块的明确诊断,但超声对多源性肿块的诊断有局限性。

Abstract:Objective:To discuss the diagnostic value of two-dimensional and colour Doppler uhrasonography in thyroid tumors. Methods:18 cases of thyroid adenoma confirmed by surgery and pathology,37 of nodular goiter and 6 cases of thyroid cancer were compared by two-dimensional and color Doppler ultrasound. Results:Adenoma,nodular goiter and thyroid cancer in the two-dimensional acoustic image chart,the coloured blood stream distribution,RI and the Vmax had the remarkable differences(P〈0.05). Thyroid gland adenoma capsule,edge halo and ring-like blood stream,multiple nodules in nodular goiter,echo change of tubercle surrounding tissues as well as the thyroid cancer low echo and calcification had high specificity in the diagnosis. But the existence of polyphyletic tumours, such as adenoma,goiter and thyroid lacked the above characteristics ,the misdiagnostic rate was high. Conclusion:The conbination of two-dimensional and color doppler ultrasonograph has high value for diagnosing thyroid tmnors clearly, but ultrasonography has limitation in the polyphyleties tumour diagnosis.
