侵犯肺静脉根部或左心房肺癌的外科治疗体会

目的:探讨累及左心房或肺静脉根部的局部晚期肺癌的手术治疗方法及疗效。方法:回顾总结1994—01~2006—12其34例肺癌累及左心房或肺静脉根部手术治疗患者的临床资料。均行左心房部分切除,其中左肺下叶切除术14例,左全肺切除术4例,右肺中下叶切除术8例,右肺下叶切除术6例,右全肺切除术2例。结果:本组无手术死亡,术后发生并发症11例,其中心律失常最为常见。术后1、3、5年生存率为83.5%、42.1%、30.1%。结论:肺癌累及部分左心房或肺静脉根部进行手术治疗是可行的,应尽可能手术治疗。

Surgery treatment for lung cancer invading the pulmonary vein root or left atrium

Objective:To explore surgical procedure for advanced lung cancer invading the pulmonary vein root or left atrium and its therapeutic effect.Methods:The clinical data on surgical procedure for 34 patients with lung cancer invading the pulmonary vein root or left atrium from January,1994 to December,2006 were reviewed.All the 34 patients received partial resection.The operations included left low lobectomy in 14 cases,left pneumonectomy in 4 cases,right middle and low lobectomy in 8 cases,right low lobectomy in 6 cases and right pneumonectomy in 2 cases respectively.Results:There was no perioperative death,11 patients had postoperative complications including arrhythmia,pneumonia and heart failure,and the 1-,3-and 5-year survival rate was 83.5%,42.1% and 30.1%,respectively.Conclusion:Surgical treatment for lung cancer invading the pulmonary vein root or left atrium is feasible and we should choose surgery as possible as we can.
