主动脉夹层假腔开口于肾动脉的腔内隔绝术

摘    要:目的 探讨胸主动脉夹层假腔开口于肾动脉的腔内隔绝方式。方法 MR评估发现胸主动脉夹层动脉瘤近端裂口位于降主动脉 ,远端裂口位于左肾动脉。采用TALENT移植物封闭近端裂口 ,自制“礼帽状”支架 人造血管复合体移植物用于封闭肾动脉裂口。结果 操作获得成功 ,两种移植物均成功植入 ,近端裂口完全封闭 ,远端裂口残余少量内漏 ,肾动脉保持通畅。结论 肾动脉内置入人造血管 金属支架复合体移植物是可行的 ,是对这种微创技术适应证的进一步扩展。

Abstract:Objective To disscuss the endovascular therapy for the aortic dissection with the distal tear at renal artery. Methods Two tears, the proximal one at the decending aorta and the distal one at the left renal artery, were revealed by MRA. Talent stent grafts were used to seal the proximal tear, while the stent graft like customed hat was used to seal the distal ones. Results Two kinds of stent grafts were deployed successfully, and the two tears were both sealed. The renal artery remained patent. Conclusion For the dissection with the tear at the renal artery, this method was fiseable and the indications of endovascular exclusion was further expanded.
