右美托咪啶对小儿鼾症手术应激反应的影响

目的:探讨右美托咪啶对鼾症手术小儿应激激素水平的变化的影响。方法:40例择期行鼾症手术的患儿, 根据是否泵注右美托咪啶,分为对照组(C组) 和实验组(D组),每组20例。在麻醉前,切皮后2 h,术后第1日抽血测定促肾上腺皮质激素(ACTH)、皮质醇和血糖(COR)。结果:2组术前ACTH,COR的值比较无统计学意义(P>0.05),而切皮后2 h及术后1 d ACTH和COR对照组明显高于右美托咪啶组(P<0.05)。右美托咪啶组血糖也较对照组低,术后2 h有统计学差异(P<0.05)。结论:右美托咪啶不仅可以安全用于小儿手术,同时能显著减轻机体应激反应。

Effects of dexmedetomidine on stress response in children undergoing uvulopalatopharyngoplasty (UPPP)

OBJECTIVE To investigate the effects of dexmedetomidine on changes of stress hormones in children undergoing uvulopalatopharyngoplasty (UPPP). METHODS Forty children between the ages of 5 and 8 years who would have selective UPPP were randomly divided into dexmedetomidine group (group D,n=20) and control group (group C,n=20). Patients in group D received dexmedetomidine, and in group C received equal volume of normal saline. Venous blood samples were collected before induction of anesthesia, 2h after operation and one day after surgery, to determine adrenocorticotrophic hormone (ACTH), cortisol (COR) and glucose. RESULTS No significant difference was observed in ACTH, COR and glucose between group D and group C before surgery. However, values of the three parameters were significantly lower in group D than group C (P<0.05). CONCLUSION Dexmedetomidine can significantly alleviate stress response in children undergoing UPPP.
