生长激素在子宫内膜发育不良中的作用

摘    要:目的:探讨辅助生殖技术中生长激素(growth hormone,GH)对子宫内膜的影响及作用。方法:回顾分析2005年1月~2007年12月因子宫内膜发育不良致体外受精-胚胎移植(in vitro fertilization-embryo transfer,IVF-ET)治疗失败66例的临床资料。再次IVF-ET治疗中,以加用GH治疗的41例为研究组,未用GH治疗的25例为对照组。结果:人绒毛膜促性腺激素(human chorionic gonadotropin,HCG)注射日研究组子宫内膜的厚度较对照组明显增加,差异有统计学意义(P<0.05),内膜的类型无明显改变。两组的Gn用量、平均获卵数、受精率、卵裂率及优质胚胎率无统计学差异(P>0.05)。HCG注射日雌二醇(estradiol,E2)水平无明显变化(P>0.05)。研究组妊娠率高于对照组(P<0.05)。结论:生长激素有助于促进子宫内膜发育,对内膜类型无明显影响。生长激素对卵子质量无显著影响,可能通过改善子宫内膜的容受性提高了妊娠率。

The effect of growth hormone on the maldevelopment of endometrium
Abstract:Objective:To explore the effect of growth hormone on the endometrium in assisted reproductive technology(ART).Methods:The clinical data of 66 patients who were failed in previous IVF-ET treatment were analysed retrospectively from Jan.2005 to Dec.2007.Maldevelopment of endometrium exisited in all the patients.In the second cycle,the 66 patients were divided into two groups,study group consisted of 41 cases with using GH and the control group consisted of 25 cases without using GH.Results:After using GH,the endometrial thickness on the day of HCG administration was thicker than that of the control group(P<0.05).There were no statistical differences between the two groups on the type of endometrium,the dose of Gn used,the number of oocytes retrieved,the rate of fertilization,cleavage and good embryos(P>0.05) and the level of estradiol(E2).After using GH,the clinical pregnancy rate was significantly improved than the control group(P<0.05).Conclusions:Growth hormone supplementation may increase the thickness of endometrium but not alter the type of endometrium.GH does not affect the quality of oocytes,while it can improve the clinical pregnancy rate perhaps through affecting the endometrial receptivity.
