2型糖尿病下肢动脉血管病变介入治疗的临床疗效观察

目的 探讨2型糖尿病下肢动脉血管病变的特点及其血管内介入治疗的临床疗效.方法 对20例2型糖尿病下肢动脉血管病变的患者行下肢动脉数字减影血管造影,并行经皮血管成形术和（或）血管内支架置入术,观察糖尿病下肢动脉血管病变的特点及手术成功率、临床疗效和并发症.结果 共治疗20条患肢48条血管,手术成功率为95.2%,临床症状改善,踝肱指数较术前明显上升,无一例严重并发症发生.结论 糖尿病下肢动脉血管病变范围广,膝以下小血管多见.介入治疗近期疗效确切,并发症少,在传统内科药物治疗的基础上,可作为一部分难治性糖尿病下肢血管病变的理想治疗方法.

Clinical effect of interventional therapy on type 2 diabetes mellitus with lower limbs arterial disease

Objectives To discuss the characteristic of lower limbs arterial disease in patients with type 2 diabetes mellitus（T2DM） and clinical therapeutic effect of interventional therapy. Methods 20 T2DM patients with lower limbs arterial disease were examined by digital subtraction angiography （DSA） and treated by percutaneous transluminal angioplasty（PTA） and（or） stenting therapy. The features of lower limbs arterial disease in patients, surjical success rate, clinical effect and complication of interventional therapy were observed. Results Different degrees of vascular stenosis or occlusion in 20 patients were observed. After intervention therapy, DSA examination showed that the degree of opening was 70 - 100% , the success rate was 95.2% , and the clincal symptoms were improved. The ABI increased significantly after surgery. No serious complication occurred. Conclusion The lower limbs arterial disease widely occurred in patients with T2DM, especially in the small blood artery below the knees. The short - term efficacy of intervention therapy with less complications was explicit , and I t could be the ideal choice for treatment of lower extremity vascular lesions based on conventional medical therapy.
