长期留置尿管患者膀胱冲洗用溶液的探讨

目的探讨不同膀胱冲洗用溶液与长期留置尿管患者尿路感染发生率的相关性。方法将90例长期留置尿管患者随机分为3组,分别用生理盐水、1∶5 000呋喃西林、0.25%氯霉素进行膀胱冲洗,比较尿液细菌培养阳性率,结合临床资料进行统计学分析。结果生理盐水组7 d、14 d、28 d尿液细菌培养阳性率分别为26.7%、40.0%、60.0%,1∶5 000呋喃西林组分别为10.0%、16.7%、20.0%,0.25%氯霉素组分别为6.7%、13.3%、16.7%,统计学分析有显著性差异（P〈0.05）。结论不同膀胱冲洗用溶液影响长期留置尿管患者尿路感染发生率,对长期留置尿管患者应做菌谱鉴定,以筛选出具有特异性的冲洗用溶液。

Exploration of various bladder flushing liquors used in patients with long-term indwelling catheter

Objective To explore the correlationship between urinary infection and various bladder flushing liquors in the patients with long--term indwelling catheter. Methods 90 patients with long-- term indwelling catheter were divided randomly into 3 groups, which received bladder flushing with physiological saline, 1：5000 fruan, and 0.25% chloromycetin respectively. Then the positive rates of urine germiculture of three groups were compared and the clinical data were analyzed combining with the former. Results The positive rates of urine germiculture of physiological saline group in 7d, 14d, 28d were 26.7%, 40.0%, 60.0% respectively; that of 1：5000 fruan group were 10.0%, 16.7%, 20.0% respectively; that of 0. 25% chloromycetin group were 6.7%, 13.3%, 16. 7% respectively. There was significant difference in the positive rates of urine germiculture among three groups （P〈0.05）. Conclusions In the patient with long--term of indwelling catheter, the occurring rate of urinary infection has correlation with various bladder flushing liquors, so we should pay attention to urine germiculture for them and choose effective bladder flushing liquor.
