腹腔镜与开腹直肠癌切除术临床效果比较

目的比较腹腔镜与开腹直肠癌切除术临床效果,探讨腹腔镜直肠癌切除术临床价值。方法随机选择120例直肠癌患者作为研究对象,随机分为腹腔镜组及开腹组各60例,手术均由同一组医师完成,并接受相同的围术期治疗;开腹组按中国抗癌协会大肠癌专业委员会所制订的手术规范进行,腹腔镜组采用气管插管全身麻醉下腹腔镜手术。结果两组手术方式、手术时间、清扫淋巴结数比较差异无统计学意义（P〉0.05）;腹腔镜组术中出血、肛门排气、留置导尿及术后住院时间、术后并发症均低于开腹组（P〈0.05）。结论腹腔镜直肠癌全系膜切除是可行和安全有效的,与开腹手术比较具有较多优点,适合在临床进一步推广应用。

Clinical effect of comparison of laparoscopic and open abdominal rectal resection for cancer

Objective To compare clinical effect of laparoscopic and open abdominal rectal resection for cancer,investigate the clinical value of laparoscopic rectal resection for cancer.Methods 120 cases of rectal cance were chose randomly as object of study,they were randomly divided into laparoscopic group and open abdominal group,60 cases in each group,the surgery was performed to complete by the same group of doctors,and accepted the same perioperative treatment;the operation was did according to making the standard by the Committee of Colorectal Cancer in open abdominal group,laparoscopic group used tracheal intubation laparoscopic surgery under general anesthesia.Results The operation methods,operation time,cleaning the lymph node number of two groups was no significant differences(P > O.05);intraoperative bleeding,anal exsufflation,indwelling catheter and postoperative hospital stay time,postoperative complications of laparoscopic group were lower than that of the open abdominal group(P < 0.05).Conclusion Laparoscopic total mesorectal excision for rectal cancer is feasible and safe and effective,there are more advantages compared with open operation,it is suitable for clinical further popularized.
