摘    要:目的应用声辐射力脉冲成像(acoustic radiation force imaging,ARFI)技术初步定量评价甲状腺弥漫性病变。方法76例甲状腺弥漫性病变患者,分为两组:原发性甲状腺功能亢进症组(primary hyperthyroidism,PHT)45人;慢性淋巴细胞性甲状腺炎组(Hashimoto’s thyroiditis,HT)31人。对两病变组甲状腺组织行ARFI技术检查,记录各甲状腺组织的剪切波传播速度(shear wave velocities,SWV)。比较两组甲状腺病变甲状腺组织的SWV。结果 PHT组甲状腺组织的SWV比HT组慢,差异有统计学意义(P=0.000)。绘制ROC曲线,以甲状腺组织SWV=2.40m/s为截点诊断HT的敏感性和特异性分别为96.7%和86.6%。结论 ARFI技术可对甲状腺组织的质地行定量检测,有助于鉴别PHT及HT。

Abstract:Objective To quantitatively assess diffuse thyroid diseases in patients using acoustic radiation force imaging(ARFI).Methods A total of 76 patients with diffuse thyroid diseases were divided into two groups:group A including 45 patients with primary hyperthyroidism(PHT);group B including 31 patients with Hashimoto′s thyroiditis(HT).ARFI was then used to measure the shear wave velocities(SWV)of thyroid lobes of the two groups.The results of the two groups were compared.Receiver operating characteristic(ROC)curve was drawn to assess the diagnostic accuracy.Results The value of SWV between the two groups was significantly different(P=0.000).The sensitivity and specificity of SWV(AUC=0.913,cut-off value=2.40m/s) in diagnosing PHT and HT was 96.7% and 86.6% respectively.Conclusion The texture of carotid throid lobes can be evaluated quantitatively using ARFI,which was useful to discriminant PHT and HT.
