骨盆骨折合并多发伤的综合救治模式的建立和临床应用

目的:探讨骨盆骨折合并多发伤的的综合救治模式的建立和临床应用效果.方法: 353例骨盆骨折合并多发伤患者按时间顺序分为常规救治组(n=172)例和综合救治组(n=181)例,常规救治组给予传统急救治疗,综合救治组给予综合救治模式治疗,对比两组患者死亡率、生存率、康复率、致残率、输血率、平均输血量、进行内外固定治疗时间.结果: 综合救治组死亡率22.7%,低于常规救治组的41.9%(P＜0.05);综合救治组康复率为82.1%,高于常规救治组的45.5%(P＜0. 05);综合救治组致残率为12. 9% ,低于常规救治组的54. 5% (P＜0. 05);综合救治组输血率为39. 2% ,高于常规救治组的24. 4% (P＜0. 05);综合救治组平均输血量少于常规救治组,且进行内外固定治疗时间短于常规救治组(P＜0. 05),差异具统计学意义.结论: 综合救治模式可提高骨盆骨折合并多发伤患者的生存率,降低患者死亡率与致残率.

Establishment and clinical application of a comprehensive treatment model for pelvic fractures with multiple injuries

Objective:To discuss the establishment and clinical application of a comprehensive treatment model for pelvic frac-tures with multiple injuries. Methods:353 patients of pelvic fractures with multiple injuries were divided into routine treatment group (n=172) and comprehensive treatment group (n=181) according to the time sequence,the routine treatment group was treated with traditional first aid treatment,and the comprehensive treatment group was treated with comprehensive treatment mode treatment. The mortality, survival rate, recovery rate,disability rate, blood transfusion rate,average volume of blood transfusion,and time of internal and external fixation were compared between the two groups. Results: The mortality rate of the comprehensive treatment group was 22. 7% ,which was lower than 41. 9% in the conventional treatment group(P＜0. 05). The recovery rate of comprehensive treatment group was 82. 1% ,which was higher than 45. 5% in the routine treatment group (P＜0. 05). The disability rate in the comprehensive treatment group was 12. 9% ,which was lower than 54. 5% in the routine treatment group (P＜0. 05). The blood transfusion rate in the comprehensive treatment group was 39. 2% ,which was higher than 24. 4% in the conventional treatment group (P＜0. 05). The aver-age volume of blood transfusion in the comprehensive treatment group was significantly less than thar in the routine treatment group (P＜0. 05),and the time of internal and external fixation treatment in the comprehensive treatment group was significantly shorter than routine treatment group(P＜0. 05),these differences were statistically significant. Conclusion:Comprehensive treatment mode of pelvic fracture combined with multiple injuries can significantly improve the survival rate of the patients and reduce the mortality and disability rate of the patients.
