彩色多普勒超声在肝移植术后的应用

摘    要:目的:探讨彩色多普勒超声在肝移植术后的应用价值。方法:运用常规二维及彩色多普勒超声检测11例移植肝的血流动力学变化及胆管、肝实质声像图改变,并对病变部位引导活检或置管引流。结果:移植后2周内,各例均有移植肝不同程度增大、实质回声增强、肝动脉血流速度减低和门静脉血流速度增高。2例肝内胆管呈轻度扩张,2周内恢复。并发胆道感染、肝脓肿各1例,各例均存在不同程度胸腹腔积液。结论:彩色多普勒超声对移植肝术后血流动力学变化及并发症的检出具有重要意义,有助于指导临床采取相应治疗措施并提高术后存活率。

Abstract:Objective To evaluate the value of color Doppler ultrasonography in monitoring liver transplantation and its complication.Methods Eleven patients after liver transplantation were examined by using Color Doppler Flow Imaging to observe the structures of liver and biliary system and the hemodynamic changes of the vascular system.Biopsy and bile ductdrainage were performed under the guidance of color Doppler ultrasonography.Results Within two weeks after liver transplantation,the liver became hyperplasia in certain degree,the echo of the liver strengthened and the intrahepatic bile duct slightly enlarged with two case and restored with two weeks.The blood flow velocities of the hepatic artery was slower than that of the normal,and the velocity of the portal vein was faster.There were one case with bile duct infection,one with left lateral liverabscess respectiely.There were pleural effusion and ascites in a certain degree.Conclusion Color Doppler ultrasonography is valuable for monitoring normal liver transplantation and its complication.
