超声引导无水乙醇注射疗法治疗肝细胞癌的临床疗效观察

目的评价无水乙醇治疗肝癌的临床疗效及不良反应,探讨该疗法的临床价值。方法病理证实的原发性肝癌72例分为病灶直径≤3cm,36例,肿瘤结节41个;病灶直径3～5cm,20例,肿瘤结节35个;病灶直径〉5cm,16例,肿瘤结节29个,在超声引导下注射无水乙醇,2～3d注射1次,4～10次为1个疗程。观察该疗法的效果。结果病灶直径≤3cm组、病灶直径3～5cm组及病灶直径〉5cm组近期有效率分别为86.1%,70.0%,68.8%,病灶直径≤3cm组、直径3～5cm组和直径〉5cm患者≥1、≥2、≥3年生存率分别为98.1%、87.4%、78.3%,89.6%、80.0%、61.3%和65.2%、46.2%、34.6%。病灶直径≤3cm及直径3～5cm肿瘤患者的≥2、≥3年生存率显著高于直径〉5cm肿瘤患者（P〈0.05）。结论超声引导注射无水乙醇治疗肝癌疗效显著,可明显提高肝癌患者的生存率和生活质量,明显延长患者的生存期。

Later Effects of Ultrasound-Guided Percutaneous Ethanol Injection for Treatment of Hepatic Carcinoma

Objective To evaluate the later effects and the significance of follow-up after PEI of ultrasound-guided percutaneous ethanol injection （PEI） for the treatment of 72 cases with hepatic carcinoma.Methods The pathologically proved hepatic carcinoma in 72 cases were treated with PEI every 2～3 days and 4～10 times per treatment course.Results The recent effects in the cases with hepatic carcinoma≤3cm,3-5cm and〉 5cm in diameter were 86.1%,70.0% and 68.8%,respectively.The survival rates of ≥1、≥2、≥3 years recurrence rates in the cases with hepatic carcinoma≤3cm in diameter were 98.1%、87. 4% and 78.3%,respectively. The survival rates of ≥1、≥2、≥3 years recurrence rates in the cases with hepatic carcinoma 3～ 5cm in diameter were 89.6%、80.0% and 61.3%,respectively. The survival rates of ≥1、≥2、≥3 years recurrence rates in the cases with hepatic carcinoma5cm in diameter were 65.2%、46.2% and 34.6%,respectively.The survival rates of ≥2、≥3 years in the cases with hepatic carcinoma≤3cm and 3～5cm in diameter were markedly higher than those of the carcinoma 〉5cm in diameter （P〈0.05）.Conclusion PEI has significant later effects on improving the survival rate of hepatic carcinoma patients to prolong patients＇life.
